Kim Wagner, LSW | Community Liaison

Kim was born and raised in Apple Valley. She attended Minnesota State University, Mankato and earned her Bachelor of Science in Social Work (BSSW). As a Licensed Social Worker (LSW), Kim has worked in a variety of settings including transitional care, long term care, and memory care units. Kim is the President of the Minnesota Nursing Home Social Workers’ Association (MNHSWA), Twin Cities Chapter.

As a community liaison, Kim is available to meet with home care clients, family members, and referral sources to provide personalized service. Her approach is always client centered. One of Kim’s philosophies is that attitude is a choice. We have a choice each day regarding the attitude we will embrace for that day—and for Kim, a positive attitude is key.

Kim is known for her excellent communication skills and empathetic approach. She possesses a deep understanding of healthcare procedures and policies, which she uses to simplify complex information for clients and their families. Her background in social work enables her to comprehend patient needs and communicate them effectively to the care team.

When not in the office, Kim enjoys spending time with family and friends at their cabin in Wisconsin. Kim also enjoys playing cribbage, reading fiction novels, and trying new restaurants.
